OPSC:3 WARNING ORDER V3

A) List of participating Members with the follwing information:
1a) Full Name
2a) FooD preference in terms of Cultural/Religious, Medical (allergies, etc.)
3a) Medical History if any
B) TEAM LOGO in Jpg Format. Should not be less than 72 dpi
send a very high resolution if you want. your team patch will be printed on the left chest of the shirt and the OPSC:3 A4 print at the back of the Shirt for the freebies.
C) individual T-shirt Sizes
S - small, M - medium, L- Large, XL - extra Large, XXL - double extra Large, XXXL
D) TEAM CONTACT PERSON with his MOBILE pHONE AND Land Line for immediate communications purposes.
All Teams must be at the SRP OPSC CAMP ON OR BEFORE 0700hRS 23 FEBRUARY 2008.
PLEASE ADVISE how many Extra T-shirts with sizes you would want us to print with your TEAM Logo in front. Extra shirts will cost PhP350 Only.

Spoke to EL Chikano today. I think I will join if i can make it just put me on reserve.
Pls don't take out anybody from the list. Dont' mind me ha. It's fine ith me if can only play 1 or 2 games in the entire event.
